Fee Schedule • San Jose Depression Counseling

The information below explains my policies and procedures for insurance reimbursement and my session fee.

Please note that each session is 45 minutes in length and the session fees listed below apply to the type of therapy/counseling that is reserved at the time the appointment is scheduled.

Fees

 

  • Individual therapy $200.00 (one person)
  • Couples counseling and family therapy $275.00 (two to three people)

 

Fees are payable by cash, check, Visa, HSA, or Master Card.

Cancellation Policy

If you need to cancel or reschedule an appointment, please do so by phone by calling 408-315-0645 with at least 48 hours notice before your scheduled appointment time. This allows my office time to offer that hour to someone else. If you do not show for your appointment, or if you cancel or reschedule with less than 48 hours notice, you will be charged the regular fee for the type of therapy/counseling session that you reserved.

Insurance

I am an out-of-network provider for PPO insurance plans. This means that the client pays the depression therapist the full fee directly to me, and I provide a statement so that you may apply for reimbursement. Most plans pay back a percentage to you, depending on your level of coverage. Some plans reimburse the full fee for a set number of sessions.

If you have a PPO plan, I recommend submitting statements for reimbursement, as it can significantly reduce your out-of-pocket expenses. Even with other plans, I recommend that you call and ask if your insurance will reimburse for out-of-network providers, and inquire what the reimbursement rate is for your individual plan.
